If you would like to have your name listed as a Dog Walker on the Champlain Park Community Association website, please read the Terms of Service agreement and complete the form below.

We will review your request and post your name and phone number within five business days. Parents will be contacted to confirm permission to publish their child’s name and contact information (telephone number).
