Please use this form to submit a Community Project Proposal (CPP) to be considered by the Champlain Park Community Association (CPCA), its Partners and Sponsors.

The submission of the CPP form initiates the start of the CPP process.
An assigned CPP coordinator will contact you within 10 days of your proposal submission.

For a proposal to be considered it must first meet the objectives outlined in the criteria and agreement of the Terms and Conditions must be given as part of completing this form.
